The Complex Care Navigator supports patients and families living with serious illness by helping them navigate the healthcare system and align medical care with the patient’s goals, values, and lived experience. This role serves as a consistent point of contact for patients and caregivers, fostering trusting relationships and helping bridge communication between patients, families, providers, and care teams. The navigator works collaboratively within an interdisciplinary palliative care team to support coordination, communication, and patient-centered care.
