About The Position

The Cardiac Nurse Navigator utilizes a systematic approach to nursing by incorporating assessment, planning, implementation, and evaluation in the coordination of care for patients with heart failure and cardiomyopathy. Working primarily in an ambulatory setting, the Cardiac Nurse Navigator provides ongoing monitoring of patient status, facilitates timely follow‑up, and supports patients and families across the continuum of outpatient cardiac care. This role collaborates closely with an interdisciplinary team, interfacing with providers to ensure seamless communication, appropriate escalation of care, and adherence to evidence‑based treatment plans. The Cardiac Nurse Navigator offers patient and family education, connects individuals with community resources, and supports advanced care planning when applicable. The Cardiac Nurse Navigator promotes quality of life and reduces avoidable hospital utilization by providing proactive care coordination within the RN scope of practice as defined by the Illinois Board of Nursing. Acting as an advocate for high‑risk patients, the Cardiac Nurse Navigator applies the nursing process to assess needs, plan interventions, implement supportive strategies, and evaluate outcomes to optimize patient health and engagement in their care.

Requirements

  • Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
  • Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Cardiovascular Nursing Certification (CV-BC) American Nurses Credentialing Center within 2 years - American Nurses Credentialing Center (ANCC)
  • Advanced Cardiac Life Support (ACLS) within 1 year - American Heart Association (AHA)
  • Bachelor's Degree: Nursing
  • Medical nursing

Responsibilities

  • Assists patients in understanding diagnosis, treatment options and resources available internal and external to the organization.
  • Serves as liaison in planning care between patients, caregivers and multi-disciplinary health care providers.
  • Provide education and informational resources to patient to optimize continuous and comprehensive care.
  • Provides and facilitates competent health care of patients and families in area of practice.
  • Ensures adherence to established standards of care and scope of practice.
  • Ensures documentation of care is complete and accurate.
  • Provides health and wellness education to individuals and groups.
  • Acts as preceptor and supports the development of other staff and healthcare students.
  • Provides leadership for initiatives designed to improve the quality of care or clinical outcomes.
  • Assists Nursing Supervisor/coordinator in the development of departmental standing orders.
  • Provides detailed heart failure education to patients in the outpatient setting following discharge.
  • Is a vital part of the ambulatory care team for the heart failure patient population.
  • Attends daily huddles to provide input and address care and educational needs for all heart failure and cardiomyopathy patients.
  • Provides follow up phone calls and care coordination, post hospital discharge, to all heart failure/cardiomyopathy patients to address ongoing needs and prevention of readmission.
  • Works closely with an interdisciplinary team to ensure comprehensive post‑discharge care and ongoing support for heart failure patients in the ambulatory setting.
