The RN Navigator collaborates with a multidisciplinary team to evaluate patient referrals and develop individualized care plans across the continuum of care. This role is responsible for creating personalized education plans for patients and their families, following up on test results, and acting as a liaison between patients, families, community resources, providers, physicians, and other healthcare team members. The RN Navigator also collaborates in developing and evaluating teaching strategies, provides clinical leadership by precepting professionals and students, and coordinates educational materials for optimal patient outcomes. Additionally, they participate in continuing education programs, monitor patient outcomes, collect and report risk stratification data, and coordinate community services and resources. The role involves serving on hospital and community committees, participating in research, and collaborating with various departments on data and reports. The RN Navigator is accountable for implementing and communicating patient care delivery standards and developing clinical tools for optimal outcomes.
