About The Position

The Nurse Manager supervises the nursing staff, medical assistants (MA), and nurse case managers who work in their team or department. Their work involves collaborating with other team leadership, and other nurse managers to ensure proper staffing, promote patient access, and improve workflows and processes to improve patient health outcomes and satisfaction. The nurse manager's role is pivotal in ensuring a safe and healthy environment that supports the healthcare team's work and contributes to patient engagement. They oversee patient care, make management decisions, and work with staff to ensure that State, Federal, and other regulatory agency guidelines for patient safety are followed. The nurse manager is also crucial in educating staff about regulatory changes affecting care delivery. Lastly, the Nurse Manager is held to the same expectations outlined in the RN (Registered Nurse) job description when providing care to patients.

  • Master’s Degree, preferred
  • Bi-lingual in Spanish /Khmer/Russian with knowledge of medical terminology preferred.

Responsibilities

  • Lead and manage the nurses and medical assistants on your team.
  • Maintain a safe environment for staff and patients, ensuring that standards and the quality of care are maintained and aligned with the organizational goals.
  • Follow the guidelines outlined in the Union contract and employee handbook when making disciplinary decisions and managing human resources.
  • Ensure that the department workflows and activities are aligned with the organization's strategic plan and motivate staff to strive for professional excellence through mentorship, coaching, and professional development opportunities.
  • Support staff by providing resources for implementation of evidence-based practices.
  • Have a basic understanding of budgeting and financial processes.
  • Delegate responsibilities based on scope of practice, education, appropriate competence, and experience.
  • Collaborate with other nurse managers and practice managers to ensure adequate staffing on the team to support patient care.
  • Ensure timely communication with the team regarding staffing, assignments, and information that may impact workflow.
  • Work closely with practice managers to ensure scheduled templates (nursing, providers, BH, etc.) are being used as intended.
  • Ensure staff collaboration between the different departments and external agencies.
  • Be an active participant at meetings such as the daily 7:30 am staffing huddle meeting, the weekly Nurse manager meeting, the monthly all-leadership meeting, and any other mandatory assigned meetings.
  • Support staff in managing departmental changes.
  • Manage staff behavior using empathy, coaching skills, and discipline guidelines outlined in the Union Contract and the Employee handbook.
  • Ensure all staff consistently follow LCHC standards of care and policies/procedures.
  • Assure the team has the necessary resources to provide patient care as outlined in their job description.
  • Promote integration of evidence-based research into practice at the unit level.
  • Work with the team to complete quality improvement projects as identified by the organization/team.
  • Conduct quarterly/yearly evaluations for all staff under direct supervision.
  • Participate in the advancement of the organization's goals.
  • Interview, hire, and coordinate the onboarding/training of new hires.
  • Respond to customer service breakdowns that occur on the team.
  • Ensure the reporting of reportable events in Compliatric.
  • Oversee the ordering of supply for the department.
  • Provide comprehensive care to clients with HIV, Hepatitis C, Latent (LTBI)or Active Tuberculosis, Hepatitis A and B and Sexually Transmitted infections.
  • Include education on disease process, risk reduction, and medication adherence.
  • Participate in direct and indirect patient care for patients who have HIV, and/or HCV and/or TB or LTBI and/or Sexually Transmitted Infections, and/or Hepatitis A or B, including in-person visits, telephone triage, assessment, wound assessment, intervention, health maintenance, health education, health counseling, and planning activities.
  • Coordinate care for patients with active TB with nurses from local public health departments and Salem Hospital.
  • Participate in meetings, calls and follow-up with the Massachusetts Department of Public Health TB Division.
  • Track and reviews laboratory and other pertinent diagnostic testing, informing medical provider of all results in a timely manner.
  • In collaboration with the ID Provider, HIV Specialist/Family Medicine provider, Community Health Worker, Case Manager, Clinical Pharmacist, other Providers, patient and family formulate a plan of care that supports best practice and guides the patient in achieving identified/revising Self-Management goals.
  • Obtain required specimens and perform or request diagnostic tests. Ensure that provider and patient are notified of abnormal results.
  • Accurately document observation, pertinent patient conversations and action in the Electronic Record and in any grant required forms/logs.
  • Collaborate with other programs serving the patient to foster seamless care.
  • Assure that patients are scheduled for follow up, lab or other pertinent visits and receive periodic phone calls to support their health maintenance activities, adherence and possible medication side effects. Provide follow up of no shows.
  • Maintain professional and technical knowledge by attending trainings and other related continuing education units.
  • Coordinate patient care with other departments or providers within the health center including social service, enrollment, referral, Community Health Counselors, BH, HIV, WIC, SBHC, and Dental etc. to assure comprehensive health services for assigned patients.
