About The Position

The Registered Nurse (RN) / Nurse Manager, under the general direction of the Administrator, is responsible for overseeing and coordinating all direct resident care within the facility. This role ensures that residents are clinically prepared and medically optimized for scheduled procedures and surgeries. The RN/Nurse Manager provides leadership and supervision to The Licensed Practical Nurse (LPN):and Certified Nursing Assistants (CNA), ensuring the delivery of high-quality, safe, and patient-centered care. Responsibilities include participating in and guiding the assessment, planning, implementation, and evaluation of resident care, while maintaining compliance with clinical standards, regulatory requirements, and organizational policies. The RN/Nurse Manager also promotes effective communication among interdisciplinary team members to support optimal resident outcomes.

Requirements

  • Associate’s Degree required; Bachelor’s degree preferred.
  • Active Connecticut Registered Nurse (RN) certification is required.
  • Active CPR Certification is required.
  • A minimum of five (5) years of RN experience and an additional two (2) years of management experience required.
  • Experience working with elderly and/or mentally ill population is required.
  • Microsoft Word, Excel, and Outlook proficiency.
  • A current driver’s license and the ability to maintain a current license.
  • Successfully pass a pre-employment physical, drug screening, educational and certification background check, extensive references and criminal background checks.

Responsibilities

  • Manages all direct patient care for the residents at Scofield Manor.
  • Maintains integrity of medication room, including medication delivery, inventory, distribution to ensure adherence to the Department of Public Health, as well as accuracy and safety.
  • Supervises patient direct care activities of daily living staff provided by the LPN and CNAs, which includes, but not limited to, training, performance reviews and improvement initiatives.
  • Ensures that designated staff schedule all medical appointments, inclusive of transportation, for (eye, dental, mental health, etc.), as well as tests, and lab work in a timely manner.
  • Addresses training opportunities and/or concerns, follows-up accordingly.
  • Completes annual performance reviews.
  • Communicates daily with staff regarding any change in medication(s) or residents’ condition(s).
  • Collaborates with residents, family, staff, physicians and other healthcare providers to ensure strong communication and the highest quality of care.
  • Communicates with residents or with required family members about all physical and mental health issues.
  • Ensures that detailed, thorough records on each resident are maintained and in adherence to state regulations.
  • Reviews various assessments, logs, etc. and follows-up where necessary.
  • Assess residents returning from hospitalization.
  • Transcribes doctor orders and implement any orders, as necessary.
  • Ensures that designated staff maintains and updates Care Plans for each resident.
  • Evaluates all resident applicants for appropriateness onsite and in the community, when necessary.
  • Arranges referrals for home care, physical therapy or any other service needed.
  • Collaborates with Scofield Manor Administrator on resident discharge planning.
  • Provides all nursing functions, such as, wound care, nebulizer treatments, diabetic testing, flu vaccines, etc.
  • Obtains and maintains signed physician orders per state regulations.
  • Obtains required documentation of appointments, including consultation required by specialists.
  • Reviews monthly assessments of blood pressure and weight of each resident or more often as needed.
  • Oversight of staff’s active certifications, including medication and CPR, as well as basic life support knowledge.
  • Supports the LPN in coordinating direct care staff development/monthly in-service training programs; maintains records for each staff member as regulated by state requirements.
  • Oversight and support; ensures accuracy of documents to support continuity of care.
  • Ensures that all medication is accounted for and systems in place are adhered to daily by the Department of Public Health.
  • Supervises, maintains, and updates medication administration records to adhere to state regulations.
  • Adheres to all local, state, federal and Charter Oak Communities’ policies, procedures, standards and codes.
  • Ensures that department is prepared for onsite, unscheduled visits from the Department of Public Health or any other related agency.
  • May be required to perform other related duties as required and/or assigned.
