About The Position

Cantex is seeking an RN/LVN Charge Nurse for Skilled Nursing at The Crescent in Sugar Land, Texas. The role participates in and oversees patient care within the scope of LVN/LPN licensure. The nurse observes, assesses, and reports patient conditions and changes, documents care in accordance with company policies and applicable guidelines and regulations, ensures care plans are implemented as directed, and administers prescribed medications and treatments. The role also participates in assigning duties and supervises CNA staff to help maintain patients’ highest practical well-being. Requirements

Graduation from an accredited nursing school.
Valid LVN/LPN (Licensed Vocational Nurse or Licensed Practical Nurse) license.

Responsibilities

Participate in and oversee patient care within the scope of LVN/LPN licensure.
Observe, assess, and report patient conditions and changes.
Document care in accordance with Cantex’s Clinical Policies & Procedures and state and federal guidelines and regulations.
Complete EMR documentation related to patient assessment, evaluation, and changes of condition in accordance with nursing practices.
Ensure implementation of care plans as directed.
Administer medications and treatments as prescribed.
Participate in assigning duties and supervise CNA staff’s delivery of clinical care to support patients’ highest practical well-being.
