RN/LPN Hospice Float - 32hrs/week
About The Position
The Float Nurse provides patient care to assigned patients in assigned regions. Duties may include single visits, referral or admission visits, telephone triage, and temporarily serving as a primary nurse to cover leaves and absences. The position is cross-trained to provide individualized nursing services to patients and families across multiple HopeWest teams and locations, and participates in the on-call rotation. HopeWest is a nonprofit organization providing hospice, palliative, and PACE services to the Western Slope. Its work focuses on patient and family care and changing community experiences of aging, illness, and grief.
Requirements
- Unrestricted RN or LPN license in the State of Colorado.
- Valid Colorado driver’s license, reliable transportation, and auto insurance coverage.
- At least 1 year of experience.

- Acute care experience is preferred.
Responsibilities
- Provide patient care to assigned patients in assigned regions.

- Provide individualized nursing services to patients and families across multiple teams and locations after cross-training.
- Participate in the on-call rotation.
