About The Position

Angels of Care Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to infants, children, and/or adolescents in their homes. The role involves focusing on the medical needs and treatment of patients according to their active plan of care, conducting ongoing assessments, administering prescribed medications and therapies, coordinating care, educating family members, and implementing preventative initiatives to ensure quality of care. The position offers full-time and part-time shifts, with days, nights, and weekends available.

Requirements

• Active RN or LPN/LVN license (New Grads Welcome, training provided!)
• Provide care in a client home setting
• Ability to make a positive and lasting impression!

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
