Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. The client is a 63-year-old man in hospice care, bed-bound, and living with his wife in Mt Pleasant. He enjoys sports, TV, and games. Key responsibilities include tube feeding, managing hypopituitarism and complex comorbidities, and monitoring for adrenal crisis risk. The home environment is supportive, neat, and smoke-free, with 3 cats. Shifts available are Saturday and Sunday, 9:00 AM – 5:00 PM.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience preferred
- Trach/vent experience a plus (training provided)
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
- Tube feeding
- Manage hypopituitarism and multiple complex comorbidities
- Monitor for adrenal crisis, particularly from stressors like infection
