Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is actively recruiting compassionate, experienced Pediatric RNs or LPNs for an urgent home care case in Auburn Hills, MI. A devoted single father is working hard to bring his 4-month-old son home before the college semester begins in September. We need dedicated pediatric nurses to help make this homecoming possible and ensure this sweet baby receives top-tier care in a loving environment. The client is a 4-month-old baby boy with clinical needs including G-tube care, oral suctioning for secretions, and low-flow oxygen monitoring. The family is looking for a warm nurse who will engage in tummy time, read books, support physical therapies, and actively participate in developmental milestones. The nurse must be able to comfortably perform transfers from the crib to the floor for play and therapy, which requires lifting. The home environment is quiet and welcoming. Requirements
- Active RN or LPN license in the state of Michigan.
- Experience in home health or private duty nursing preferred.
- Trach/Vent knowledge is helpful, but we will train motivated candidates!
- A compassionate heart and a commitment to patient-centered care.
- Must be able to comfortably perform transfers from the crib to the floor for play and therapy (lifting required).
Responsibilities
- Provide skilled nursing care (Trach/Vent management, G-Tube/GJ-Tube feedings, medication administration).
- Monitor and document vitals and symptoms within our integrated EMR system.
- Assist with ADLs (repositioning, hygiene, and mobility).
- Collaborate with families and the clinical team to ensure superior patient outcomes.
