This role involves monitoring the Electronic Health Record nurse pool, triaging incoming calls, messages, and In-Basket tasks, and scheduling appointments as needed. The position requires exercising nursing judgment and utilizing standard triage guidelines to meet patient health needs. The nurse will assess complex patient needs, ensure appropriate transitions of care, and collect and assess patient data. Responsibilities include triaging and scheduling visits, providing clinical support, answering phone calls and electronic messages promptly, educating patients about community services, and collaborating with the interdisciplinary care team. Additional duties include greeting and escorting patients, obtaining vital signs, performing medication reconciliation, assisting clinicians with procedures, resolving patient concerns, participating in meetings and training, staying current with community resources, maintaining patient rights and confidentiality, and providing clerical support such as scheduling, registering patients, scanning records, and preparing medical records. The role may require floating to other MACC practices and verifying clinician's orders for tests and procedures. The LPN position requires graduation from an accredited Practical Nursing program, licensure in Massachusetts, and Basic Life Support Certification. The RN position requires graduation from an accredited school of nursing, licensure in Massachusetts, and Basic Life Support Certification. Both roles require strong interpersonal, customer service, and time management skills, proficiency with computer applications including Electronic Medical/Health Records and MS Office, and a minimum of two years of experience in a clinical setting is preferred.
