About The Position
Intake Registered Nurse (RN) – Home Health [Stuart] | Office-Based Position | Monday–Friday Review Referrals. Coordinate Care. Help Patients Start Their Home Health Journey. We are seeking an experienced Intake RN to support our clinical operations team by reviewing referrals, coordinating admissions, and ensuring patients receive timely access to care.
Requirements
- Active Florida RN License
- Home health experience strongly preferred
- Knowledge of Medicare home health regulations
- Strong organizational and communication skills
- Experience with EMR systems (HCHB preferred)
- Ability to prioritize and manage multiple referrals

Responsibilities
- Review incoming home health referrals for clinical appropriateness
- Communicate with hospitals, physicians, case managers, and referral partners
- Verify eligibility and services ordered
- Coordinate admissions and start-of-care scheduling
- Support clinicians with referral and intake questions
- Ensure compliance with Medicare and agency guidelines
- Assist with care coordination and patient transitions
