Intake Assessment Coordinator - RN
About The Position
Brentwood Hospital is seeking a dedicated Admissions Nurse - RN to join their team, focusing on providing help and hope to individuals and families in improving their mental health. The Registered Nurse Assessment and Referral Counselor will conduct telephonic and in-person risk assessments and crisis interventions, serve as the initial contact for referral sources and potential patients, manage the referral and intake process, determine appropriate levels of care, facilitate admissions, and maintain communication with all involved parties. Brentwood Hospital is a 260-bed psychiatric and chemical dependency treatment facility with over 45 years of service, catering to children, adolescents, adults, and seniors. Requirements
- Associates or Bachelor's Degree in Nursing
- Currently licensed to practice by the Louisiana State Board of Nursing as a Registered Nurse.
- Prior intake, assessment, admission and crisis intervention experience with a psychiatric mental health population, preferable in an inpatient setting.
Nice To Haves
- Prefer knowledge of crisis intervention and risk assessment, psychiatric diagnoses symptomatology, basic chemical dependency, age specific growth and development and crisis behavior management.
Responsibilities
- Perform initial medical screening
- Perform admission assessments
- Assess, direct and intervene in crisis situations
- Perform nurse to nurse screening by phone on patients being transferred from other facilities
- Obtain/document pre-certification on admissions as necessary
- Basic Medical Triage: skin check, wounds, etc.
