About The Position

The PRN Registered Nurse-Flex RN team has opportunities to practice any one of Hospice of Acadiana, Inc.’s direct patient care departments in areas such as Clinical, Admissions, and Inpatient Unit. Individual assignments are determined by staffing volumes to serve identified areas with the greatest needs. Director of Nursing and/or Clinical Supervisor will communicate the staffing needs to the Flex RN Pool on a weekly or monthly basis. Each shift will be assigned to staff on a first come, first response basis.

Requirements

Valid Louisiana Registered Nurse.
Associate’s degree; Conferred graduate of an accredited nursing school.
Minimum two (2) years’ experience as a Registered Nurse.
Possesses valid Louisiana Driver’s License.
Automobile Liability Coverage.
BLS CPR cert.

Nice To Haves

Hospice experience preferred.

Responsibilities

Complete an initial, comprehensive admission and ongoing assessments of patient and family to determine hospice needs.
Provide a complete physician assessment and history of current and previous illness(es).
Provide professional nursing care by utilizing all elements of nursing process.
Assess and evaluate patient’s status by writing and initiating plan of care, re-evaluating patient and family/caregiver needs and can participate in revising the plan of care when necessary.
Use health assessment data to determine terminal nursing diagnosis and create plan of care that establishes goals, based on diagnosis and incorporates palliative nursing actions, including the patient and family in the process.
Initiate appropriate preventative and rehabilitative nursing procedures.
Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
Counsel the patient and family in meeting nursing and related needs.
Assist the patient with the activities of daily living and facilitate the patient’s efforts toward self-sufficiency and optional comfort care.
Act as Case Manager, Admissions Nurses, or Inpatient RN when assigned by Director of Nursing, Clinical Supervisor, and/or Clinical Coordinator and assume responsibility to coordinate patient care for assigned caseload/assignment.
Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given.
Record pain/symptom management changes/outcomes as appropriate.
Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders as required.
Communicate with community health related persons to coordinate the care plan.
Teach the patient and family/caregiver self-care techniques as appropriate.
Provide medication, diet and other instructions as ordered by the physician and recognize and utilize opportunities for health counseling with patients and families/caregivers.
Work in concert with interdisciplinary team and with the family/caregiver to meet the physician emotional needs of the patient.
Provide and maintain safe environment for the patient.
Triage phone calls during assigned shifts pertaining to caseload/assignment.
Ensure that arrangements for Durable Medical Equipment (DME), supplies and other necessary services are available to patient/caregiver.
Supervise ancillary staff and delegate responsibilities when required.
Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in on-going education and in-service classes (Relias).
Fulfill obligations of requested and/or accepted assignments.
Fulfill after-hour and holiday assignments as assigned by Clinical Coordinator.
PRN Flex Pool will be required to attempt to schedule or sign up for at least two shifts in any role per month and one holiday shift per year. If the shift is canceled due to not needing PRN staff, the shift will count towards the requirement.
