About The Position

ComForCare is hiring a Registered Nurse to manage home care clients in Fort Collins, Greeley, and neighboring Colorado communities. The RN Case Manager oversees a client caseload, conducts in-home assessments, develops and updates care plans, and guides caregivers in delivering client-centered care. The role includes coordinating with families, physicians, hospitals, and community partners; maintaining compliant documentation; and responding to client and caregiver concerns. The position offers independence and ongoing support in a home care setting.

Requirements

Active, unencumbered Registered Nurse (RN) license in Colorado.
At least 1 year of home care or home health nursing experience.
Current BLS/CPR certification.
Valid driver’s license and reliable transportation.
Oasis documentation knowledge.
Strong clinical judgment, organization, and communication skills.
Passion for client-centered care.

Case management, care coordination, or caregiver supervision experience.

Responsibilities

Complete initial and ongoing in-home nursing assessments of clients’ physical, cognitive, psychosocial, and home safety needs.
Create and update individualized plans of care and ensure caregivers understand and follow them.
Manage a caseload of home care clients, monitor changes in condition, and adjust services as needed.
Conduct caregiver supervisory visits and competency checks.
Coordinate care as the main clinical contact for families, physicians, hospitals, and community partners, and make referrals when needed.
Maintain accurate, timely client records and follow Colorado home care regulations, infection control, and HIPAA guidelines.
Provide on-call support to resolve client and caregiver concerns promptly and professionally.
