About The Position

ComForCare is hiring an experienced Registered Nurse to manage a caseload of home care clients in Greeley, Colorado, and neighboring cities. The role offers the independence of home care while providing support from a certified Great Place To Work® organization. The RN leads client care, guides caregivers, and builds relationships with clients and families. The position is full-time, pays $47–$52 per hour based on experience, and is scheduled Monday through Friday on day shifts from 8 a.m. to 5 p.m., with flexible scheduling, occasional holidays, and a weekend rotation.

Requirements

Active, unencumbered Registered Nurse (RN) license in Colorado.
At least 1 year of home care or home health nursing experience.
Current BLS/CPR certification.
Valid driver’s license and reliable transportation.
Oasis Documentation.
Strong clinical judgment, organization, and communication skills.
Passion for client-centered care.

Nice To Haves

Case management, care coordination, or caregiver supervision experience.

Responsibilities

Complete initial and ongoing in-home nursing assessments of clients’ physical, cognitive, psychosocial, and home safety needs.
Create and update individualized plans of care and ensure caregivers understand and follow them.
Manage a caseload of home care clients, track changes in condition, and adjust services as needs change.
Conduct caregiver supervisory visits and competency checks.
Serve as the main clinical contact for families, physicians, hospitals, and community partners, and make referrals when needed.
Maintain accurate, timely client records and follow Colorado home care regulations, infection control requirements, and HIPAA guidelines.
Provide on-call support to resolve client and caregiver concerns quickly and professionally.
