About The Position

The RN Hospice role is centered on providing compassionate, patient-focused nursing care to individuals facing life-limiting illnesses, ensuring their comfort and dignity during end-of-life stages. This position requires the nurse to assess, plan, implement, and evaluate individualized care plans in collaboration with interdisciplinary teams, patients, and their families. The RN Hospice nurse plays a critical role in symptom management, emotional support, and education, helping patients and families navigate complex medical and psychosocial challenges. This role also involves advocating for patient needs and preferences while maintaining compliance with hospice care regulations and standards. Ultimately, the RN Hospice nurse contributes to enhancing the quality of life for patients and supporting families through a holistic, empathetic approach to care.

Requirements

  • Current and valid Registered Nurse (RN) license in the United States.
  • Minimum of one year of nursing experience, preferably in hospice, palliative, or home health care settings.
  • Basic Life Support (BLS) certification.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently and manage a caseload effectively.

Nice To Haves

  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
  • Experience with electronic medical records (EMR) systems.
  • Advanced certifications such as Advanced Cardiac Life Support (ACLS).
  • Experience in patient and family education and counseling.
  • Familiarity with Medicare and Medicaid hospice regulations and documentation requirements.

Responsibilities

  • Conduct comprehensive patient assessments to determine physical, emotional, and psychosocial needs.
  • Develop, implement, and regularly update individualized care plans tailored to patient goals and comfort.
  • Administer medications and treatments as prescribed, monitoring patient responses and adjusting care accordingly.
  • Provide education and emotional support to patients and their families regarding disease progression, symptom management, and end-of-life planning.
  • Collaborate closely with interdisciplinary teams including physicians, social workers, chaplains, and volunteers to coordinate holistic care.
  • Document all patient interactions, care plans, and clinical observations accurately and timely in compliance with regulatory standards.
  • Participate in quality improvement initiatives and maintain up-to-date knowledge of hospice care best practices and regulations.
