About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Per diem Hospice Registered Nurse provides compassionate, patient-centered care by assessing patient needs, coordinating individualized care plans, managing symptoms, educating families, and collaborating with the clinical team. The role includes assigned on-call responsibilities and services to patients and families.

Requirements

• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Nice To Haves

• At least 1 year of nursing experience.

Responsibilities

• Performs initial nursing assessments for new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care in collaboration with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records accurately, promptly, and according to care center policies.
• Records changes and outcomes in pain and symptom management as appropriate.
• Educates families about the patient's condition, needs, and care.
• Makes referrals to other hospice care disciplines as needed, or documents the rationale for not doing so.
• Performs on-call responsibilities and provides on-call services to patients and families as assigned.
• Performs other duties as assigned.
