About The Position
The Hospice Admissions RN coordinates new patient admissions, completes comprehensive assessments, educates patients and families, and works closely with the interdisciplinary care team to support a smooth transition into hospice services.
Requirements
- Current Registered Nurse license.
- Current driver's license.
- Current CPR certification.
- Two years of Registered Nurse experience in a clinical care setting.
- One year of full-time hospice experience.
- Reliable transportation and valid automobile liability insurance.
Responsibilities
- Coordinate and oversee all aspects of new patient admissions.
- Collaborate with patient care teams to support seamless communication and continuity of care.
- Educate patients and families regarding condition, needs, and care.
- Perform head-to-toe physical assessments.
- Complete all required documentation accurately and on time.
