About The Position

The Hospice Admissions Registered Nurse coordinates new patient admissions and helps ensure a smooth transition into hospice services. The nurse completes comprehensive and head-to-toe assessments, educates patients and families about conditions, needs, and care, and works closely with the interdisciplinary care team to support continuity of care. The position requires accurate and timely completion of documentation.

Requirements

• Current Registered Nurse license.
• Current driver's license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding conditions, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
