RN-Hospice Admission Nurse
About The Position
Tapestry Hospice & Palliative Care is a family-owned and operated organization serving North Georgia for over 16 years. We are seeking exceptional individuals to join our growing team, fostering a compassionate and supportive work environment. As an Admissions Nurse, you will be an integral part of the Interdisciplinary Team, conducting skilled nursing assessments as needed, in accordance with Tapestry Hospice policies and procedures. You will collaborate with patients, caregivers, the clinical manager, case managers, and physicians to develop an initial plan of care and serve as a liaison among team members throughout the admission process.
Requirements
- Current RN License in the State of Georgia
- Valid Georgia driver’s license, insurance and reliable vehicle
- Excellent Time Management Skills
- Current RN License
Nice To Haves
- 1-2 years nursing experience
- 1-2 years hospice experience (other candidates may be considered with experience in skilled nursing, home health, acute care, medsurg or ICU)
- Utmost compassion & integrity
- The ideal candidate would live in or around the Rome area
Responsibilities
- Provide skilled nursing assessments on an as-needed basis consistent with Tapestry Hospice policies & procedures.
- Collaborate with the patient, caregivers, clinical manager, case managers, and physician to establish an initial plan of care.
- Act as a liaison between various team members during the admission process.
