About The Position

As a locally owned and operated hospice, our team is deeply connected to the community and dedicated to walking alongside patients and families during one of life's most important chapters. We are seeking a skilled and compassionate Hospice Admissions Registered Nurse (RN) to join our clinical team. The Admissions RN plays an important role in welcoming new patients and families into hospice care by completing comprehensive assessments, coordinating with providers, and helping ensure a smooth transition into services. The ideal candidate is clinically confident, compassionate, organized, and comfortable communicating with patients, families, physicians, and interdisciplinary team members. This position is an excellent opportunity for a nurse who enjoys making a meaningful difference from the very beginning of a patient's hospice journey.

Requirements

  • Graduate of an accredited school of nursing.
  • Current and valid RN license in the State of New Mexico.
  • Minimum of 1 year of clinical nursing experience.
  • Strong assessment, communication, organization, and clinical decision-making skills.
  • Valid driver's license and current automobile liability insurance.
  • Ability to work independently while remaining an effective member of an interdisciplinary team.

Nice To Haves

  • Previous hospice, home health, or palliative care experience.
  • Previous hospice admissions experience.
  • Experience with electronic medical records.
  • Knowledge of hospice regulations, eligibility requirements, and Medicare guidelines.

Responsibilities

  • Conduct comprehensive hospice admissions and initial nursing assessments.
  • Evaluate patients to determine their clinical needs and eligibility for hospice services.
  • Collaborate with physicians and the interdisciplinary team to develop appropriate plans of care.
  • Educate patients and families about hospice services, goals of care, medications, and available support.
  • Provide compassionate guidance and support to patients and families during the transition to hospice.
  • Assess symptoms and identify immediate care needs requiring appropriate intervention or follow-up.
  • Coordinate with healthcare providers, facilities, referral sources, and members of the hospice team.
  • Complete accurate, timely, and compliant clinical documentation.
  • Ensure a smooth and compassionate transition from referral to hospice admission.
  • Communicate effectively with patients, families, physicians, and interdisciplinary team members.
