About The Position

Join our compassionate team at Amedisys Hospice of Brevard in Melbourne, FL! We’re part of one of the nation’s largest and most trusted home health and hospice organizations, dedicated to providing exceptional care and support to patients and families. This position is based in Melbourne, FL and requires daily travel within Brevard County. We offer a competitive salary range of $85,000-92,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills. We also offer a fleet vehicle program with mileage reimbursement. Benefit eligibility can vary by position depending on shift status and requires enrollment in an Amedisys medical plan.

Requirements

  • Current RN license, specific to the state(s) you are assigned to work.
  • Minimum 2+ years of clinical RN experience in acute healthcare setting(s)
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
  • Current or previous hospice or home health experience in the last 3 years.

Responsibilities

  • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
  • Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
  • Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Performs other duties as assigned.
