RN-Hospice Admission Nurse
About The Position
Tapestry Hospice & Palliative Care is seeking an RN-Hospice Admission Nurse to join their team. This role involves providing skilled nursing assessments as an active member of the Interdisciplinary Team, consistent with Tapestry Hospice policies and procedures. The Admissions Nurse will collaborate with the patient, caregivers, clinical manager, case managers, and physician to establish an initial plan of care and act as a liaison between team members during the admission process. Requirements
- Current RN License in the State of Georgia
- Valid Georgia driver’s license, insurance and reliable vehicle
- Excellent Time Management Skills
- Current RN License
Nice To Haves
- 1-2 years nursing experience
- 1-2 years hospice experience (other candidates may be considered with experience in skilled nursing, home health, acute care, medsurg or ICU)
- Utmost compassion & integrity
- The ideal candidate would live in or around the Rome area
Responsibilities
- Provide skilled nursing assessments on an as-needed basis consistent with Tapestry Hospice policies & procedures.
- Collaborate with the patient, caregivers, clinical manager, case managers, and physician to establish an initial plan of care.
- Act as a liaison between various team members during the admission process.
