About The Position

Rocky Mountain PACE is seeking an RN to provide skilled nursing assessments to participants. This position is responsible for working directly with the Interdisciplinary Team (IDT), participant, and his or her representative to achieve the goals of the care plan. The RN will communicate with the family or caregiver and provide appropriate patient education.

Requirements

• Graduate of an accredited Registered Nurse Program.
• Licensed as a professional in the State of Colorado.
• Reliable transportation.
• Current automobile insurance.
• Familiarity and efficient work with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records.
• Knowledge of medical equipment and instruments.

Nice To Haves

• 1 year of experience in a hospital, long-term, or home health care setting.
• Experience with a frail or elderly population.
