About The Position
CommonSpirit Health at Home is hiring a PRN Home Health Registered Nurse in Houston. The role provides personalized nursing care in patients’ homes, with a 1:1 patient-clinician ratio and time for patient education, assessment, and support. The nurse guides patients throughout the healing process, adapting care plans and supporting recovery. The position uses NestMed ambient AI technology and has no after-hours on-call requirements. Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of nursing experience.
- Exceptional critical-thinking skills and proven clinical expertise.
- Strong case-management abilities and commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

- Home Health experience is a significant plus.
Responsibilities
- Deliver high-quality, personalized nursing care in patients’ homes.
- Develop, implement, and adjust comprehensive individualized care plans to promote patient independence and well-being.
- Coordinate care with physicians, therapists, and support staff as part of a multidisciplinary team.
- Educate and support patients and their families, including providing emotional support.
- Manage a caseload and oversee, lead, and mentor LPNs, LVNs, and Home Health Aides.
- Use technology and best practices to optimize care delivery and maintain accurate documentation.
