About The Position
Sage Group HealthCare Staffing is seeking an RN- Home Health for a travel/contract position in Littleton, NH. This role involves providing all aspects of nursing care for an assigned caseload of home health patients, completing assessments and tailored plans of care for both Home Health and Hospice patients. The nurse will care for patients at all stages of life in their homes, working as part of a multi-disciplinary team with a high level of autonomy. Documentation will be completed as assigned. Strong clinical support and educational resources are provided. The territories covered include the Littleton/Plymouth areas.
Requirements
- Registered Nurse experience: 1.5 to 2 years - Required
- Current experience Home Health RN experience: 1.5 years – Required
- Must have valid Driver’s License and Personal Auto Insurance
- RN License for State of NH or Compact - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- Prior Hospice experience in addition to home health experience - Preferred
- Prior Med Surg experience - Preferred
Responsibilities
- Provide all aspects of nursing care for home health patient case load assigned.

- Mileage Reimbursement: $0.70/mile for distance covered from office to patient home visits and in between patient home appointments
