Requirements
- Active RN license
- 1+ year nursing experience, Home Health preferred
- Strong assessment and critical thinking skills
- Ability to work independently in the field
- Reliable transportation and willingness to travel within territory
- Ability to frequently lift, push, pull, and support up to 50 pound
Nice To Haves
- HomeCare HomeBase experience preferred
Responsibilities
- Manage your own caseload from admission through discharge
- Complete Start of Care, OASIS, and routine visits
- Develop and adjust individualized care plans
- Educate patients and families to improve outcomes
- Partner with physicians and interdisciplinary teams
- Document efficiently using HomeCare HomeBase
