You manage your caseload, build meaningful relationships, and practice with independence while still having strong clinical support. We care for more than 60,000 patients each day. Requirements

• Active RN license
• 1+ year nursing experience
• Strong assessment and critical thinking skills
• Ability to work independently in the field
• Reliable transportation and willingness to travel within territory
• Ability to frequently lift, push, pull, and support up to 50 pounds

Nice To Haves

• Home Health or Hospice experience preferred
• HomeCare HomeBase experience is a plus

Responsibilities

• Manage your own caseload from admission through discharge
• Complete Start of Care, OASIS, and routine visits
• Develop and adjust individualized care plans
• Identify changes in patient condition and act quickly
• Educate patients and families to improve outcomes
• Collaborate with physicians and interdisciplinary teams
• Document efficiently using HomeCare HomeBase
