About The Position

The Home Care Case Manager (HCCM) or Visit Nurse provides and coordinates all components of home care services for patients. The nurse provides care as ordered by the physician, follows applicable policies, procedures, and conditions of participation, and regularly reports to a supervisor. The role facilitates development and implementation of the physician-outlined plan of care, completes documentation related to care delivered, and communicates pertinent patient information to appropriate members of the healthcare team. This is a full-time Weekend Warrior position in the Central South territory, working every Friday and Saturday in 12-hour shifts. The role uses the Home Care Home Base documentation system. A $7,500 sign-on bonus is advertised.

Requirements

Graduate of an accredited school of nursing; an associate or bachelor's education is listed.
Current Ohio Registered Nurse license in good standing.
One to three years of job-related experience.
IV skills required.
Clinical experience must have occurred within the last three years unless currently employed in a home care setting.
Must be capable of standing, stooping, bending, and lifting up to 40 lbs.
Must be self-directed and able to work with little supervision.

Nice To Haves

One year of clinical experience in an acute care setting preferred.
Computer skills preferred.
Home care experience preferred.
For specialist work, documentation of skills and certifications must be provided.
