About The Position

The RN Home Based Primary Care position is a 100% FTE, day-shift role with a hybrid work arrangement. The in-person work location is 2550 Charles St. The position requires a graduate of an approved School of Nursing program, an associate's degree in nursing, and a current Illinois RN license. The nurse must have at least one year of professional nursing experience within the past three years in an acute care setting, as well as a valid driver's license and Basic Life Support/CPR certification.

Requirements

• Graduate from an approved School of Nursing program.
• Associate's degree in nursing.
• Provide a copy of a high school diploma or equivalent, or the highest level of completed degree obtained.
• At least 1 year of experience as a professional nurse within the past 3 years in an acute care setting.
• Current RN license from the State of Illinois.
• Current, valid driver's license issued by the individual's state of residence.
• Basic Life Support/CPR certification.

• Experience in home health, IV therapy, ER, or oncology.
