About The Position

The RN Home Based Primary Care position is a 100% FTE, day-shift role with a hybrid arrangement. The in-person work location is 2550 Charles St. The role requires a current Illinois RN license, a valid driver’s license, and Basic Life Support/CPR certification. Applicants must have at least one year of professional nursing experience within the past three years in an acute care setting. Experience in home health, IV therapy, the emergency room, or oncology is preferred. Requirements

Graduate from an approved School of Nursing program.
Associate’s degree in nursing.
Provide a copy of a high school diploma or equivalent, or the highest level of completed degree obtained.
At least 1 year of experience as a professional nurse within the past three years in an acute care setting.
Current RN license from the State of Illinois.
Current, valid driver’s license issued by the applicant’s state of residence.
Basic Life Support/CPR certification.
