About The Position

The Miami VA Healthcare System is seeking an RN for its Geriatric/Long Term Care division. This role involves providing services across three units: long-term care, short-stay, and hospice/palliative care. The position requires the nurse to be responsible and accountable for all aspects of the nursing process, including direct patient care, assessment, planning, implementation, and evaluation of care based on age-specific needs. The role also focuses on coordinating care transitions, educating patients and families, supporting self-management post-discharge, and enhancing customer satisfaction. Responsibilities include managing daily living activities, administering medications and procedures, performing IV therapy, coordinating with interdisciplinary teams, handling admissions and discharges, managing transfers, delegating tasks, documenting in electronic medical records, patient transport, and responding to emergencies.

Requirements

  • Completed courses for nursing from an accredited program approved by either the Commission for Education in Nursing or the Commission on Collegiate Nursing Education.
  • A current license in at least one state, territory, or commonwealth in the U.S. or the District of Columbia.
  • A minimum of three years' direct patient care experience.
  • Must have Basic Life Support certification.

Responsibilities

  • Provide services in the long-term care division comprised of 3 long-term care units; long-term, short-stay, hospice/palliative care residents.
  • Be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Assess, plan, implement and evaluate care based on age-specific components.
  • Assume responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
  • Be responsible for patient activities of daily living, assessments, medication, administering medication and procedures per established policies and guidelines, ion administration, IV therapy, interdisciplinary team coordination, admissions, and discharges, internal and external transfers, delegating to assistive personnel, electronic medical record documentations, patient transport, and emergency response procedures.
