About The Position

Houston Hospice is seeking a PRN RN Field Case Manager to manage patients’ cases and coordinate care with patients, families, and other providers. The role provides nursing case management and supports development and implementation of patient care plans with the multidisciplinary team. Houston Hospice is an independent nonprofit hospice serving the Greater Houston area and provides care in patients’ homes, long-term care facilities, and its inpatient care unit.

Requirements

• Graduate of an accredited school of nursing and hold a current Texas nursing license.
• At least one year of hospice experience or experience in a hospital setting.
• Current basic life support certification.
• Ability to work as part of an interdisciplinary team.
• Strong interpersonal skills and ability to communicate with team members and patients from a diverse community.
• Reliable transportation or a reliable insured vehicle and a valid Texas driver’s license.
• Ability to travel to multiple work locations throughout the day.

Nice To Haves

• Bachelor’s degree in nursing preferred.
• Knowledge of the hospice industry and experience in a hospice or hospital setting strongly preferred.
• Experience using a clinical information system preferred.

Responsibilities

• Provide direct nursing care and case management appropriate for a licensed RN.
• Participate in developing patients’ plans of care and coordinate multidisciplinary team services and activities in accordance with those plans.
• Document activities and assessments in the clinical information system.
• Support patients’ families and friends through education, setting expectations, and reporting changes in condition.
• Comply with organizational policies and processes related to regulatory, accreditation, and privacy standards.
• Perform other duties as required or requested by management.
