The Registered Nurse - Community Housing & Behavioral Health Services provides comprehensive nursing services across TGCW Behavioral Health programs and Community Housing programs throughout Westchester County. The RN supports individuals with mental health, substance use, and co-occurring medical conditions in clinic, urgent care, community, residential, and scattered-site housing settings. This position coordinates population-based healthcare activities, nursing assessment, triage, medication management, health education, disease prevention, infection control, care coordination, and wellness support. The RN collaborates with psychiatrists, therapists, medical providers, housing staff, care managers, and community partners to ensure continuity of care and regulatory compliance. The RN serves as a clinical resource for staff, supports individuals across lifespan, facilitates access to medical and behavioral health care, and promotes recovery-oriented, trauma-informed services in both housing and behavioral health settings.
