About The Position

Voyage Healthcare is seeking a Registered Nurse (RN) for a Clinical Support/Triage role in an ambulatory clinic setting. The RN will utilize a systematic approach to nursing practice, including assessment, planning, implementation, and evaluation, in accordance with the Minnesota Nurse Practice Act and Voyage Healthcare policies. This role involves close collaboration with providers and care teams to coordinate patient care, delegate responsibilities, and promote patient safety. Key duties include conducting nurse visits, administering immunizations/injections, assisting with procedures, patient education, and triaging symptom-based walk-in patients. The RN will also assess patient needs over the phone, provide medical guidance, and determine the necessary level of care based on symptoms and established protocols. This position is crucial for ensuring patients receive timely, accurate, and compassionate support, and for helping them access care efficiently and safely.

Requirements

  • Graduate of an accredited school of nursing required
  • Current Registered Nurse (RN) license in the State of Minnesota required
  • Current CPR certification required
  • Excellent communication and interpersonal skills
  • Ability to interpret medical records, physician orders, and medication instructions
  • Strong organization skills and ability to work independently and collaboratively
  • Commitment to outstanding customer service

Nice To Haves

  • Minimum of 2 years nursing experience preferred

Responsibilities

  • Maintain a respectful and professional work environment
  • Deliver quality nursing care in a patient-focused manner
  • Manage incoming patient calls using established protocols and gathering relevant clinical information
  • Provide ambulatory nursing care across various specialties
  • Implement care according to policies, procedures, and protocols, and best-practice standards
  • Assist with or perform medical procedures and treatments
  • Recognize and respond to medical emergencies; perform CPR as needed
  • Provide culturally competent health education and counseling
  • Accurately document care, communication, and outcomes in the Electronic Medical Record (EMR)
  • Promote a safe, equitable, and respectful environment
  • Maintain compliance with OSHA standards and workplace safety policies
  • Perform duties such as: Medication and immunization visits, Stand-alone RN visits, Telephonic and in-person triage, Care coordination/case management, Prescription refills per protocol
