About The Position
This is an opportunity for a passionate nursing leader to make a meaningful impact in home health. PruittHealth @ Home is seeking a Registered Nurse (RN) – Clinical Manager to oversee clinical operations and patient care services in their home health division. The role involves leading with compassion, driving excellence, and being part of a supportive team.
Requirements
- Active, unrestricted RN license in the state of practice.
- Current CPR certification.
Nice To Haves
- Minimum of two years RN experience
- At least one year of Home Health experience
Responsibilities
- Provide daily clinical oversight of patient care services.
- Coordinate referrals and admissions with internal and external sources.
- Evaluate patient needs and agency capabilities before admission.
- Supervise care planning, partner assignments, and care delivery.
- Ensure compliance with regulatory standards and agency policies.
- Mentor and coach clinical staff to promote morale and retention.
- Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.
