This role coordinates clinical program development activities to support the strategic growth goals for the TVC Clinic. It is responsible for understanding market data analysis, planning, implementation, maintenance and evaluation of initiatives to increase volume to services and programs. The role works to integrate services and outreach activities to maximize efficiency and increase patient referrals. It develops, implements and evaluates a plan to maintain excellent relationships with referring physicians and facilities. The role works collaboratively with designated medical leaders, program directors, managers and staff to develop, implement and evaluate outreach goals. It provides patient education and coordinates all aspects of new patient referrals to medical oncology. The RN Clinical Intake Coordinator is the main resource for all referring physicians to medical oncology and provides and implements care coordination of patients through the continuum of care with the medical oncologist.
Requirements
- One-year registered nursing experience, or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration.
- Current applicable state RN license.
- AHA Basic Life Support (BLS) for Healthcare Providers.
Nice To Haves
- Oncology Certified Nurse (OCN) certification preferred.
Responsibilities
- Coordinates clinical program development activities to support the strategic growth goals for the TVC Clinic.
- Responsible for understanding market data analysis, planning, implementation, maintenance and evaluation of initiatives to increase volume to services and programs.

- Provides and implements care coordination of patient through the continuum of care with the medical oncologist.
- Performs and documents bio-psychosocial assessments according to patient age and condition.
- Formulates and implements a plan of care for each patient which includes interventions, evaluation of response to interventions and documentation as appropriate to condition and age.
- Plans and coordinates nursing care and clinical decisions using the best available evidence.
- Promotes safe practice as it relates to patient care.
- Recognizes, intervenes, and reports patient safety issues/risks.
- Prevents transmission of communicable diseases in the healthcare setting.
- Complies with all laws, regulations, scope of practice guidelines, and Legacy standards in providing patient care.
- Interactions with every individual demonstrate consideration, dignity, respect, and with cultural sensitivity.
- Communicates and obtains information while respecting the rights of privacy and confidentiality of information.
- Includes family and family dynamics in care planning and cultural competency.
- Identifies patient/client education needs, assesses learning readiness and barriers to learning, creates/obtains, appropriate educational materials, teaches patient/client and evaluates effectiveness of education.
