About The Position

MercyOne is seeking an RN- Clinical Coordinator for their MercyOne Iowa Heart Advanced Heart Failure Clinic. This role coordinates comprehensive, specialized care for heart failure patients during their inpatient stay at MercyOne Des Moines Medical Center. The coordinator collaborates with various hospital services, including the hospitalist program, critical care program, and cardiovascular service line, to educate and manage patient care across the continuum. A primary goal is to prevent heart failure readmissions by working with a multidisciplinary team, including hospitalists, cardiologists, pharmacists, social workers, and the paramedicine program, to ensure a smooth transition from inpatient to outpatient care.

Requirements

  • Currently licensed by the State of Iowa as a registered nurse with 5 year acute care or advanced heart failure care
  • Bachelor’s degree required.
  • Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
  • Basic Life Support (BLS) for the Healthcare Provider certified or obtained by the end of the orientation period (approximately six (6) weeks). Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers.

Responsibilities

  • Coordinates and manages patient care through the continuum of the patient’s hospital stay; including but not limited to evaluations, patient education and engagement, treatment, and scheduling discharge follow-up care, including post hospital Transitional Care Management (TCM) call within two business days after hospital discharge.
  • Medication reconciliation
  • In collaboration with the organization’s hospitalist, critical care cardiovascular teams, and other multidisciplinary departments, develops an evidence-based comprehensive plan of treatment for patients with HF through clinical pathways, algorithms, and protocols
  • Serves as a HF resource to all members of the healthcare team, patients and their families.
  • Ensure compliance with CMS standards, national guidelines, patient satisfaction, and cardiac registry for best practice
  • Works with physicians and staff to effectively manage referrals and process patients.
  • Educate and correspond with outlying facilities and community services regarding care of HF patients
