About The Position

Edward M. Kennedy Community Health Center is seeking a RN Clinical Care Manager for their Medical Assistant Treatment Program (MAT) based in Worcester, MA. The Clinical Care Manager (CCM) will provide, coordinate, and organize evidence-based care management for MAT patients. This role is crucial in supporting patients in their healthcare journey, ensuring they receive optimal care, education, and preventive services across various settings. Requirements

  • Registered Nurse License in MA
  • Bachelor’s degree in Nursing from an accredited college or university
  • 1-2 years of clinical experience
  • Current CPR certification
  • Most recent COVID-19 booster and the yearly flu vaccine

Responsibilities

  • Assisting Primary Care Providers (PCPs) in coordinating and managing the care of MAT program patients, ensuring they receive optimal care, education on lifestyle changes, and preventive care across multiple settings.
  • Managing care plans for patients transitioning between healthcare facilities, aiming to prevent disease exacerbation, improve outcomes, and reduce hospital and ER visits.
  • Identifying and coordinating care, maintaining communication with the care team on patient status and needs.
  • Providing direct patient care, including assessments, vaccinations, medication reconciliation, and lab reviews.
  • Involving patients and support systems in care planning and education.
  • Assisting in creating self-management plans and documenting patient care and progress in the EHR.
  • Evening, weekend, and holiday shifts may be required.
