Responsible for developing, implementing, and evaluating programs and systems that support quality services and ongoing program accreditation. Collaborates with interdisciplinary partners to achieve program goals. This role involves serving as an expert practitioner, role model, and clinical resource in providing quality nursing care to program-specific patients and their families. The coordinator supports the care team in clinical management, follows patients through their continuum of care, and assists in ensuring coordinated, cost-efficient care with optimal outcomes. They also monitor compliance with program-specific standards, coordinate multidisciplinary team services, and provide and document program-specific care. A key aspect is identifying the need for and coordinating the development and implementation of care paths for program-specific patients.
