About The Position

Amedisys is hiring a registered nurse for a full-time, day-shift hospice case management role. The nurse provides care to patients at home, assesses new admissions, coordinates and updates individualized plans of care, tracks patient progress and symptom management, and supports patients’ families through education and referrals. The role also includes assigned on-call responsibilities.

Requirements

• Current RN license specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Nice To Haves

• One year of nursing experience preferred.
• Applicants with less than one year of nursing experience may be considered for the RN new graduate program.

Responsibilities

• Performs initial nursing assessments on new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care in collaboration with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains accurate and timely individualized patient care progress records according to care center policies.
• Records pain and symptom management changes and outcomes as appropriate.
• Educates families about the patient’s condition, needs, and care.
• Makes referrals to other hospice care disciplines as needed, or documents the rationale for not doing so.
• Performs assigned on-call responsibilities and provides on-call services to patients and families.
• Performs other duties as assigned.
