About The Position

Serenity Hospice is seeking a dedicated and compassionate RN Case Manager to provide care, comfort, and support to hospice patients and their families. The role develops individualized care plans, conducts regular check-ups, and communicates with family members as needed. The position oversees the Bergen, Morris, and Passaic County area and is based in Paramus, New Jersey.

Serenity Hospice provides hospice care services throughout New Jersey with a mission to alleviate pain and symptoms, foster calmness and harmony, and encourage appreciation of family, friends, and life experiences.

Requirements

• Current New Jersey RN license in good standing.
• Willingness to travel to assigned areas.
• Reliable transportation with liability insurance.
• Strong communication and interpersonal skills.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice care needs.
• Regularly reevaluate patients’ nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to direct the team’s work and provide continuous, coordinated care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with the physician regarding patient needs and report changes in the patient’s condition.
• Supervise Hospice Aides and LPNs.
