About The Position
PurposeCare is seeking a Home Care Registered Nurse Case Manager to provide home-based care. The role offers flexible scheduling, manageable caseloads, independent clinical decision-making, and opportunities to build one-on-one relationships with patients in their homes. Nurses provide skilled visits addressing patients’ needs and supporting progress in daily life and recovery. Requirements
- Current license with the state Board of Nursing.
- At least 1 year of nursing experience.
- Ability to perform physical tasks that may include lifting up to 50 pounds, bending, and standing.
- Current driver’s license and vehicle insurance, and/or access to dependable transportation.
- Ability to successfully complete a background check.
