About The Position

Fairview has an immediate opening for an RN Case Manager to join our team at the Vascular Clinic. This position will travel to our vascular clinics in Maplewood, Woodbury, and Wyoming, as well as our surgery centers in Minneapolis and Maple Grove. The Vascular Center Case Manager RN works in a variety of different roles and clinics in the department, focusing on case management of Vascular, Vein and Wound patients, ensuring all patient needs are met and providing education. The role involves triaging nursing calls, conducting independent wound and education visits, and collaborating with physicians, LPNs, CMAs, and patients.

Requirements

Associates Degree in Nursing
Basic Life Support (American Heart Assoc or Red Cross)
Registered Nurse MN

Bachelors Degree in Nursing
1 year Wound Care experience
Acute Care experience
Vascular Certification

Responsibilities

Under the direction of the Vascular Center physician/nurse practitioner, prepares and implements a care plan according to the Vascular Center written orders and updates appropriately.
Provides ancillary patient care needs to include, but not limited to review of laboratory and culture data; appropriate interaction with physicians and pharmacies; triaging of telephone calls; interaction with caregivers.
Completes and documents wound and vascular assessment on both new and follow-up patients for each patient visit.
Assess patient's educational needs.
Provides communication between the Center and ancillary departments and care givers to facilitate compliance with vascular and wound plans of care including orthotics, physical medicine, etc.
Implements wound and vascular patient care protocols where positive viral screens occur.
Coordinates the ordering of clinical supplies and PDWHF as needed.
Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.
Actively participates in creating and implementing improvements.
Performs other responsibilities as needed/assigned.
