About The Position

Hinds LifeCare, a nonprofit agency specializing in hospice and palliative care, is seeking a Full Time RN Case Manager. This role involves providing skilled palliative and end-of-life care in the community. The RN Case Manager will be part of the Interdisciplinary Team, developing collaborative care plans and coordinating hospice services for patients and their families. The position emphasizes promoting a quality of life and connection with loved ones during a patient’s final days.

Requirements

• Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
• One year experience in acute care facility or community health agency.
• Current RN Licensure and CPR Certification.
• Valid driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
• Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.).
• Available to work 8:00am-4:30pm M-F with on call rotation.
