About The Position

Moments Hospice is seeking an RN Case Manager to serve as the clinical lead for patients and families receiving hospice care. The role focuses on managing symptoms, coordinating care, and supporting families through end-of-life transitions. It is field-based, Monday through Friday, from 8:30 AM to 5:00 PM. Requirements

• Hold an active Registered Nurse (RN) license in the state where the employee works.
• Demonstrate proficiency with electronic medical records (EMR).
• Have strong organizational, communication, and critical-thinking skills.
• Be able to work independently while maintaining high standards of care.
• Bring a compassionate, patient- and family-centered approach.
• Have a valid driver’s license and a reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at the end of life.
• Coordinate care across the caseload and serve as the primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
