About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide one-to-one, whole-person care to patients and families in their homes. Working with an integrated care team, the RN Case Manager coordinates individualized care plans, supports families through complex decisions, and provides comfort with professionalism, empathy, and respect. The role offers meaningful patient connections and autonomy in practice. This is a full-time position based in Grand Rapids, Minnesota, with a scheduled workday of 8:00 a.m. to 5:00 p.m. and rotating on-call duties. The role requires 100% travel. The stated annual base salary range is $80,000–$96,000; actual salary depends on factors including location, education, certifications, experience, and skills.

Requirements

Associate, bachelor's, or master's degree from an accredited nursing program (RN).
At least 1 year of recent acute care experience.
Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

Hospice or home health experience preferred.

Responsibilities

Complete initial and ongoing comprehensive assessments to determine hospice needs.
Develop individualized care plans with patients and families and update them as needs evolve.
Administer prescribed medications and treatments.
Educate patients and caregivers on care techniques, medications, diet, and symptom management.
Rotate on-call duties and ensure seamless care transitions.
Provide one-to-one, whole-person care to patients and families in their homes.
Coordinate individualized care plans in partnership with an integrated care team.
Support families through complex decisions and provide comfort with professionalism, empathy, and respect.
