About The Position

Moments Hospice is hiring a full-time, field-based RN Case Manager to provide compassionate hospice care. The RN Case Manager serves as the clinical lead for patients and their families, managing symptoms, coordinating care, and providing support and guidance during end-of-life transitions. The role includes a weekday schedule of Monday through Friday, 8:30 AM to 5:00 PM. A fleet car program provides a company vehicle for business and personal use, with fuel and insurance covered at minimal cost, subject to eligibility.

Requirements

• Hold an active Registered Nurse (RN) license in the state where the employee works.
• Be proficient with electronic medical records (EMR).
• Have strong organizational, communication, and critical-thinking skills.
• Be able to work independently while maintaining high standards of care.
• Demonstrate a compassionate, patient- and family-centered approach.
• Have a valid driver's license and a reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across the caseload and serve as the primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to support comprehensive, holistic care.
