About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The RN Case Manager assesses patients’ needs, implements and recommends changes to individualized plans of care, coordinates with patients, caregivers, physicians, and other healthcare professionals, and supports coordinated care through participation in Inter-Disciplinary Group meetings. The role includes assigned on-call responsibilities, timely documentation, and compliance with applicable laws, regulations, and agency policies.

Requirements

Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
Current state Registered Nurse license.
Current driver’s license and reliable transportation.
Provide ongoing valid and current auto liability insurance.

Nice To Haves

Two years of experience as an RN, preferably in a healthcare or clinical setting.

Responsibilities

Complete initial and ongoing comprehensive assessments of how terminal disease affects patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal considerations.
Implement the individualized plan of care and recommend revisions as necessary.
Coordinate care among patients and caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group meetings and coordinate and participate in patient care planning, prioritization of needs, and collaboration to promote coordinated care.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
